Do some salary research beforehand and give a wide range. Otherwise, ask the interviewer what salary and compensation package they offer before answering.

This is a tricky topic because it really depends on the line of work. My friend who worked in a hotel was up for a salary increase (ie: could ask for one) every 3 months at her evaluation interview.
When I worked retail (and didn't bother to ask for a salary increase as it was a part-time, temp position), I was given one every 6 months regardless.
Promotions usually depends on who leaves, and how much time you've put into the company. I think after 6 months, you could vie for the position but you would definitely need to face it as a "new position interview" altogether (you will probably be competing against other applicants outside the company).
Hope that helps.
