As a manager in retail for 10 years and manufacturing for 5, I have developed an ability to adapt or alter who I am as a manager to meet the needs of the employee in order to work successfully together, communicate, and be productive. In my experience, I have learned that people work most productively in environments which provide them with opportunities to satisfy their motivational needs. So I always have asked the question... "What does this person need?" My greatest accomplishment is my ability to identify the primary motivational needs of any individual and create the sort of climate and communications which will cause him/her to become motivated. In other words, encourage them to want to move in the direction of achieving organizational goals while at the same time achieving their own personal goals.
EXAMPLE 1: A person who is highly extroverted tends to get their self confidence from others feeling positive about them. So they need social acceptance, opportunity to sell him/herself, and are generally motivated by recognition, spotlight, praise and attention.
EXAMPLE 2: Some people are patient and others are impatient… an impatient person with relatively higher levels of tension who may be restless, tense, high strung, etc. is generally someone who can be motivated if you can give them what they need… variety, change of pace, freedom from repetition, freedom to change priorities, etc. Whereas a person who is patient or someone who has relatively low levels of tension generally gets their self confidence from knowing the process and from affiliation. Their pleasure is predictability and their pain is unpredictability. The motivators would be… giving them process, training time, eliminate surprise and remove pressure.
EXAMPLE 3: A person who is Formal has confidence that comes certainty from doing things right. Their pleasure is perfection and pain is failure. Their needs are… certainty understanding exactly what the rules are; Time to train, to study, to dig and see the finished product; Recognition for error free work… They are generally motivated by clear expectations, performance feedback fairness, and punishments.

I don't think this has to be a work-related "accomplishment". Choose an accomplishment that you feel strongly and passionate about because it's more of a personal revelation of your character.
I would say that my greatest accomplishment was moving out to New York and living, paying for my own finances and expenses at the age of 20. It may not be an accomplishment that comes with a plaque, but it helped shape my character and build the integrity of my being than a graduate degree from a school.
I think true accomplishments are personal, and comes from a deeper more meaningful struggle than mainstream "this job" & "this school".

I did not prepare anything for the interview. I simply summed up and repeated wats put on my cv in term of work experiences and degree. I was testing the interviewer’ s reaction. Obviously she did not like it. She knew I did not do enough homework, and wasting her time. I was rejected :(

My greatest accoplishment was to mould myself from a child to a man. This happened at the age of 18 when i finshed my schooling and joined a maritime academy. Faced the racism of a different country whilst away from my loved ones. Following the hardship of a maritime academy, went at sea for a long lasting period of 8 months. Suffering a series of natural calamities including the storms of the US Gulf. Apart from the hardships of living at sea, studied for a promotion at sea and also for a long distance bachelors degree. I personally consider this as an accomplishment but if ever given a chance, i would do it all over again because it made me what i am today. A confident, independent and respected individual.
